When submitting a request for admittance, one's application must be accompanied by documentation stated below. If family members will accompany applicant, the following documents must be submitted for those members also. Furthermore, all documentation not in Dutch or English must be translated into either of said languages.

Employee requirements
One (1) recent passport picture bearing good likeness to the holder thereof;
Copy of passport;
Birth Certificate;
Certificate of good conduct not older than 6 months;
Medical certificate not older than 2 months;
Copy of Marriage Certificate / Book if applicant is married ;
or an Agreement of Concubinage made up an acknowledged notary if employee is living together with someone or Certificate of Bachelorhood / Spinsterhood not older than 6 months
Copies of diplomas and certificates of applicants;
A 5 Guilder stamp on the declaration of Guarantee from the future employer

Employer requirements
Copy of the passport, residence permit and registration form of interested party;
Copy of Business and Director's License for current year
Copy of registration form excerpt from the Chamber of Commerce for current year
Proof of labor registration
